Case study of removing a benign birthmark located on the lower eyelid of the right eye

A congenital pigmented lesion located on the lower eyelid of the right eye, in close proximity to the lash line, was successfully treated via laser excision with minimal scar. The procedure was completed over two sessions, with a total cost of approximately HK$19,500 -26,500.
To ensure ocular safety during treatment, the patient was fitted with an autoclave-sterilised stainless steel internal eyeshield, designed to protect the globe from laser-induced injury.
Preoperative Lesion Description:
1. Location: Lower eyelid, right eye; upper border in contact with the lower eyelid margin. 2. Shape: Round 0.9cm. 3. Color: Homogeneous black pigmentation. 4. Elevation: Approximately 0.2 cm above skin level. 5. Hair Growth: None observed. 6. Borders: Well defined and sharply demarcated.

The reason for the minimal scarring is due to the segmented CO₂ laser mole removal technique. There were two separate procedures, spaced two months apart.

🩺 Surgical Challenges

Because the mole was extremely large and had very deep roots, each procedure resulted in a deep wound, requiring extensive care and cleaning. Proper wound management demanded significant time and attention.

📍 Case Overview

A patient presented with a large mole located just beneath the right lower eyelid, dangerously close to the lash line. Due to its size and depth, the mole extended into the dermal layer, making removal particularly challenging. A single excision would have left a deep, wide wound and a prominent scar—especially risky in such a delicate facial area. Moreover, an atrophic scar could pull on the lower eyelid skin, causing eyelid eversion. This not only affects appearance but may also prevent the eyelid from fully closing, leading to corneal dryness and necrosis. If the patient were to lose vision, it would be a tragic outcome in the field of aesthetic medicine.

🛠️ Surgical Strategy: Staged CO₂ Laser Removal

Dr. Chung recommended a two-stage CO₂ laser procedure, spaced two months apart, to minimize trauma and optimize healing.
-First Procedure: The lower half of the mole was carefully removed, leaving a deep wound. Healing was closely monitored, and after two months, the scar was already faint and well-integrated into the surrounding skin.
