Clinical Features: Two Moles on the Right Side of the Face
Patient Information
- 65‑year‑old female
- Lesions located on the right side of the face: one beside the nasal bridge, and one on the cheek beneath the eye.
Mole Beside the Nasal Bridge
- Color: Lighter brown.
- Size: Relatively larger, more noticeable due to its central facial location.
- Morphology: Dome‑shaped, mildly raised.
- Surface: Smooth, with clear borders.
- Clinical Significance: Prominent because of its size and position; excision must be carefully performed to avoid irregularity of the nasal bridge contour.
Mole on the Cheek Beneath the Eye
- Color: Darker brown.
- Size: Relatively smaller, though still protruding.
- Morphology: Nodular, smooth surface.
- Clinical Significance: Located in a cosmetically sensitive area under the eye; overly deep excision may result in depressed scarring or local contour deformity.
Overall Clinical Features
- Both lesions are protruding melanocytic nevi, clinically benign with no ulceration, bleeding, or irregular pigmentation.
- Cosmetic concern is the main indication for removal.
- Anatomical challenges include thin skin and high mobility in these regions, requiring precise depth control during excision to avoid depressed scarring.
CO₂ Laser Excision by Dr. Chung King Lueh
The two protruding moles on the right side of the patient’s face were excised using a CO₂ laser. The laser was applied in a controlled ablative mode, carefully vaporizing the lesions layer by layer until the roots were completely removed. This ensured both cosmetic improvement and prevention of recurrence.
Because the defects were moderately deep and located in cosmetically sensitive areas (near the nasal bridge and beneath the eye), the wounds were sutured immediately after laser excision. Fine sutures were used to approximate the wound edges precisely, minimizing tension and reducing the risk of depressed scarring.
Postoperative course:
- Sutures were removed 2 weeks after surgery, once the wounds had stabilized.
- Healing progressed smoothly without infection or wound breakdown.
- At 6 months post‑operation, the scars were almost invisible to the naked eye, blending naturally with the surrounding facial skin and showing no evidence of depression.
This case highlights how precise depth control with CO₂ laser combined with meticulous suturing can achieve excellent cosmetic outcomes, even in high‑visibility areas of the face.
